Youth Permission Slip

I hereby give my permission for my child, listed above, to accompany Victory Baptist Church on  all activities during the 2022 calendar year. 

In giving my permission and selecting "Yes", I release all liablity from Victory Baptist Church, all chaperones of the activity, leaders of Victory Baptist Church and it's Youth Ministry in the case of any accidents or injuries that may occur on the church property or the property that the activity is located. 

I also release the property owners of the location of the activity from any and all liability in the case of an accident or injury. 

I affirm that I have read the guidelines below and will do my part in assuring that my child(ren) know them and abide by them during each activity.

  • Proper Attire For Gentlemen

    -For visitation- Polo or button down shirt with nice jeans or chino/khaki style pants


    -For Activities- Loose fitting pants worn at the waist, loose fitting shorts that come to the knee when sitting, t-shirt or sweat shirts (cannot promote worldly music, tv shows, alcohol or tobacco products). No piercings are allowed to have jewelry in them.

  • Proper Attire for Ladies

    -For Visitation- Loose fitting, knee length or longer skirt, modest top


    -For Activities- loose fitting, knee length (while sitting) coulottes, skirts, or boy's athletic shorts ( must be loose fitting and knee length when sitting). All shirts should be modest (cannot promote worldly music, tv shows, alcohol or tobacco products) No piercings allowed to contain jewelry other than ears. 

  • Behavior Guidelines:

    All young people on the activity should listen to and respect all authorities and leadership.


    All young people should keep their hands to themselves to avoid conflicts unless a game or activity requires  it. 


    There should be no physical contact between members of the opposite sex unless an activity or game requires it.


    No one should be out of sight of a leader or chaperone during the activtiy. 


    No boys and girls should be alone during the activity at anytime.


    No young person should not touch property of another young person while on the activity. 


    Use of perverse or foul language is not permitted. 


    Listening to ungodly or worldly music is not permitted. (no earbuds or headphones please).


  • Other Considerations

    -Outings that include both an activity and visitation: Young people should dress appropriately for visitation and they will be given an opportunity to change before the activity begins if they desire to do so.


    These guidelines my be altered on outings that include other churches or youth groups that may have more stringent guidelines than our own out of respect to that church.

REMEMBER: As a Christian we are to look and act in a manner that will bring honor and glory to our Lord and Savior Jesus Christ. We must never do or say anything to shame the name of Christ. We should always strive, in every area of our lives, to please Him and be more like Him. We are His representatives to the lost and dying world around us and we maybe the only Bible some people "read".
